Why this topic
Delirium is one of the most common — and most under-recognised — complications of hospitalisation in older adults. It is independently associated with longer length of stay, functional decline, institutionalisation and mortality, yet a large share of episodes are precipitated or worsened by medication. Anticholinergic burden, benzodiazepines, sedative-hypnotics, opioids, corticosteroids, antipsychotics and polypharmacy in the setting of age-related pharmacokinetic change all feed directly into risk. For the pharmacist reviewing an internal medicine ward list, delirium is therefore not a neurology problem but a medication-safety problem.

- Definition, subtypes (hyperactive, hypoactive, mixed) and why hypoactive delirium is so frequently missed on the wards
- Screening and assessment tools used at the bedside, and how the pharmacist contributes to detection
- The high-risk medication classes: anticholinergics, benzodiazepines, Z-drugs, opioids, antihistamines, corticosteroids, and the cumulative anticholinergic burden concept
- Applying deprescribing frameworks and explicit criteria (Beers, STOPP/START) to the older inpatient
- Non-pharmacological prevention bundles and where pharmacological management is genuinely indicated — and where it is not
- Practical medication-reconciliation and counselling points at admission, transfer and discharge
