The Saudi Geriatrics Society invites healthcare professionals to an online webinar titled "Beyond Hospital Discharge: Optimizing Transitions of Care and Post-Acute Care at Home", held on Monday 28 September 2026 from 08:00 to 09:00 PM Saudi time (1:00–2:00 PM US Eastern). The session runs for one hour and carries 1 hour of CME credit, and it is delivered live over Zoom, so colleagues can join from anywhere in the Kingdom or abroad without travel.
The topic sits at one of the most consequential points in the care of older adults: the moment a patient leaves hospital. Transitions of care — from the ward to home, to a rehabilitation facility, or to home-based post-acute services — are where medication errors, missed follow-up, functional decline and avoidable readmission most often occur. Older patients are particularly exposed, because multimorbidity, polypharmacy, frailty, cognitive impairment and dependence on family caregivers all compound the risk in the first days and weeks after discharge.
The webinar is aimed at physicians and other health professionals who look after older adults across the continuum of care: geriatricians, internists, family physicians, hospitalists, residents and fellows in training, nurses, pharmacists, physiotherapists, occupational therapists, social workers and discharge planners, as well as clinicians working in home healthcare and post-acute or long-term care services. The session is equally relevant to anyone involved in designing discharge pathways, transitional care programmes or hospital-at-home services in Saudi hospitals.
The Saudi Geriatrics Society runs a regular programme of accredited online educational activities for the geriatric medicine community in Saudi Arabia, covering clinical, service-design and caregiver-facing topics. Its webinars are short, focused, practice-oriented sessions designed to fit around clinical commitments, typically running for an hour in the evening Saudi time and followed by a live question-and-answer discussion with the audience.
Attendance is free and access is through the Zoom meeting link published by the society, with a QR code on the official invitation for quick joining from a mobile device. Participants should join a few minutes before the start time to confirm audio and video, and are encouraged to bring questions from their own practice to the discussion at the end of the hour.
